Information for Your First Visit
Patients’ first visit to our office typically consists of check-in, exams mostly including X-ray, and consultation with a doctor to hear the diagnosis and discuss treatment options. Often treatment will be provided immediately following the consultation but the patients with complex medical history or conditions may have to make another or multiple appointments for treatment.
Please alert the office if you have medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
For new patients, at the check-in, you will be asked to provide us four patient forms including medical history. To save time at check-in, we offer an option to fill out forms on-line and send electronically ahead of your visit. Alternatively, you may fill out, print, and bring the forms to your appointment. You can also fill out the forms at your visit.
Information to Bring to Your Appointment
Please assist us by providing the following information at the time of your consultation:
- A referral slip from another doctor, if applicable.
- Any x-rays taken by your previous dentist or physician, if applicable; you may request your current dentist or physician to forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they can be taken at our facility.
- A list of medications you are presently taking.
- Deental insurance information; This will allow us to process any claims in timely manner and save time.
Patient Under 18 Years Old
For patients under 18 years old, a parent or guardian must accompany the patient at their first visit to our office.
