Information for Your First Visit

Patients’ first visit to our office typically consists of check-in, examinations (X-rays), and consultation with a doctor to hear your diagnosis and discuss treatment options. Often, treatment will be provided immediately following the consultation but patients with a complex medical history or condition may have to make another or multiple appointments for treatment.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

Patient Forms

For new patients at check-in, you will be asked to provide us with four patient forms with one of them including medical history. To save time at check-in, we offer an option to fill out forms online and send them electronically ahead of your visit.  Alternatively, you may fill out, print, and bring the forms to your appointment. You can also choose to fill out the forms during your visit. 

Information to Bring to Your Appointment

Please assist us by providing the following information at the time of your consultation:

  • A referral slip from another doctor, if applicable.
  • Any X-rays taken by your previous dentist or physician, if applicable; you may request your current dentist or physician to forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional X-rays are necessary, they can be taken at our facility.
  • A list of medications you are currently taking.
  • Dental insurance information;  This will allow us to process any claims in a timely manner.

For Patients Under 18 Years Old

For any pediatric patients, a parent or guardian must accompany the patient on their first visit to our office.
